1.   I am over 21 years of age

2.   I am not under indictment in any court for a crime for witch the judge could imprison me of more than one year.

3.   I have not been convicted in any court for a crime for which the judge could have imprisoned me for more than one year, even if the judge actually gave me a shorter sentence.

4.   I am not a fugitive from justice.

5.   I am not an unlawful user of, or addicted to any controlled substances or prescription medications.

6.   I have not been discharged from the Armed forces under dishonorable   conditions.

7.   I have never been adjudicated mentally defective and I have never been committed to a mental institution.  

8.   I am legally in the United States.

9.   I have never renounced my Untied States Citizenship.

10. I am not subject to a court order restraining me from harassing, stalking or threatening an intimate partner or child of such a partner.

11. I have not been convicted in any court of a misdemeanor crime of domestic violence.  This includes any misdemeanor conviction involving the use or attempted use of physical force committed against a current or former spouse, parent or guardian or against a person with a similar relationship with the victim.

12. I have not been convicted of a crime of violence as stated in the MN Statue 624.712.
